According to the Family Education Rights and Privacy Act (FERPA) Federal law and the Texas Public Information Act, certain
information about District students is considered directory information and will be released to anyone who follows the procedures for
requesting the information unless the parent or guardian objects to the release of the directory information. If you do not want NISD to
disclose your child’s directory information without your prior written consent, you must notify the district in writing by the end of the first
10 days of instruction. Failure to return this form will result in the release of directory information upon request.
A parent is allowed to record their objection to the release of all directory information on one or more specific category of directory
information. NISD has designated the following information as directory information:

Photograph, participation in officially recognized activities and sports, and weight/height of athletic team members

if she/he won an award, or in any panoramic pictures.
Publications Outside The District

Media Release
Northside Independent School District
has
my permission to showcase my child’s achievement, award, honor, scholarship, recognition, and/or
participation in a school activity/event by allowing the NISD Communications Department and/or Staff Development Department to interview,
photograph, and/or videotape my child, either individually or as part of a group. In doing so, your child’s image and/or full name could appear in social
media communication methods (Facebook, Twitter, etc.), on the NISD website, on an NISD YouTube channel, or utilized at a local, state, or national
conference. In addition, NISD may also provide the story, image, classroom video and/or student’s name to the news media.
Artwork Display
During the school year, your child’s artwork may be selected for displays in classrooms, hallways, cafeterias, display cases, other school settings, and
various district events including contests. If your child’s artwork is selected, please understand that the art teacher will do what is suitable for the display
of the artwork. This may include matting, mounting, or temporary framing. A printed label stating student’s first and last name, grade (K-12), school, art
teacher’s name, artwork title, artwork media, and district name (NISD) will be included. While on display, the work may be photographed and used for
publicity purposes. If artwork is posted on the NISD web pages, the child’s last name will not be posted. NISD
has
my permission to display my child’s
artwork in the above district supported initiatives and for possible publicity purposes.
Military
Federal law requires districts receiving assistance under the Elementary and Secondary Education Act of 1965(20 U.S.C. Section 6301 et seq.) to provide
a military recruiter or an institution of higher education, upon request, with the name, address, and telephone number of a secondary student unless the
parent has advised the District that they do not want the student’s information disclosed without the prior written consent. NISD
has
my permission to
provide directory information to be released to a military recruiter.

Free Application for Federal Student Aid (FAFSA)
NISD has been selected to participate in the U.S. Department of Education’s (USDE) FAFSA Completion Project and will also be participating in the
TxCAN project. These projects are designed to assist schools in determining which of their senior students have not completed a FAFSA form for the
upcoming school year. In order to participate in the project, NISD is required to disclose personal information of seniors to these organizations. NISD

Acceptable Use of the District’s Technology Resources
(Please reference Sections B and E of the Student-Parent Handbook.)

I have read the Student and Parent Agreement for the Acceptable Use of the District's Technology Resources, District policies and the Administrative
Regulation found in Sections B and E of the Handbook, In consideration for the privilege of my child using the District's Technology Resources, and in
consideration for having access to the public networks, I hereby release the District, its operators, and any institutions with which they are affiliated from
any and all claims and damages of any nature arising from my child's use of, or inability to use the system, including, without limitation, the type of
damage identified in the District's policies and administrative regulation.
I give permission
for my child to participate in the District's Technology Resources, utilizing a District-provided device and/or a personal wireless or
mobile device, and certify that the information contained on this form is correct.
If permission is given, the student must read the following Student section.

devices, District online services, nor a personal device at school during this time.

All NISD campuses are able to use the Internet's rich resources, and also publish information on the Internet. As part of the curriculum, the school may
wish to publish a variety of student products on the Internet. The types of products may include, but are not limited to: creative writing, artwork, slide
and audio/visual presentations produced by the students and their teachers.
We will not publish student grades, test scores, home addresses, phone numbers, or other information protected by federal regulations.

is needed for any of the following to be published by Northside ISD:

The Administrative Regulation for Acceptable Use addresses the District’s policies and guidelines on web publication restrictions and privacy of minors

Ley de Privacidad y Derechos Educativos de la Familia (FERPA)

En conformidad con la Ley de Privacidad y Derechos Educativos de la Familia (FERPA), que es una ley federal, y la Ley de
Información Pública de Texas, cierta información acerca de los estudiantes del Distrito es considerada información del directorio y se dará a conocer a cualquiera que siga los procedimientos para solicitar la información, a menos que el padre o tutor se oponga a que se divulgue la información del directorio. Si no desea que el Distrito Escolar Independiente de Northside divulgue información del directorio de los registros educativos de su hijo(a) sin su consentimiento previo y por escrito, debe notificar al Distrito por escrito antes de que finalicen los primeros 10 días de clases. Si no regresa este formulario, la información del directorio se divulgará si es solicitada.

Comunicados de prensa
El Distrito Escolar Independiente de Northside tiene mi permiso para exhibir los logros, premios, honores, becas, reconocimientos y/o participaciones de mi hijo(a) en una actividad/un evento escolar permitiendo que el Departamento de Comunicaciones de Northside y/o el Departamento de Desarrollo de Personal entrevisten, fotografíen, y/o graben a mi hijo(a), ya sea individualmente o como parte de un grupo. Al hacerlo, la imagen y/o el nombre completo de su hijo(a) podrían aparecer en métodos de comunicación de medios sociales (Facebook, Twitter, etc.), en el sitio en la red de Northside, en un canal de YouTube de Northside o usados en una conferencia local, estatal o nacional. Además, el Distrito de Northside también puede proveer la historia, imagen, video del salón de clases y/o el nombre del estudiante a los medios de comunicación.
Exhibición del arte
Durante el año escolar la obra de arte de su hijo/a pudiera ser seleccionada para exposición en aulas, pasillos, cafeterías, vitrinas u otros sitios escolares, o eventos del distrito, incluyendo concursos. Si se selecciona la obra artística de su hijo/a, queremos que sepa que el maestro/a de arte hará todo lo adecuado para exhibir la obra de arte. Esto pudiera incluir enmarcar, montar y desmontar, o encuadrar usando molduras temporales. Se incluirá un rótulo impreso que incluirá el nombre y apellido del alumno, el grade (K-12), el nombre de la escuela y del maestro de arte, el título de la obra, medios utilizados para crear la obra y el nombre del distrito, NISD. Mientras permanezca en la exposición, la obra pudiera ser fotografiada y utilizada para propósitos de publicidad. Si la obra se coloca en las páginas web de NISD, no se pondrá el apellido del estudiante. NISD tiene mi permiso para exhibir la obra de arte de mi hijo/a en las iniciativas del distrito mencionadas anteriormente y para posiblemente para fines de publicidad.

. Families who do not have Internet access or prefer a hard copy can receive one at their
campus on request. Please complete the requested information below and return it to your child’s teacher.
