Ectopic sebaceous glands in the esophagus

The discovery of sebaceous glands in ectoderm-derived tissues such as the oral cavity, parotid glands, and external genitalia is a frequent finding. On the contrary, the presence of sebaceous glands in the esophagus, which is an organ of endodermal ori- gin, is rare: there are fewer than thirty cases reported, most of them affecting the esophagus in a patchy pattern.

CASE REPORT

A 50-year-old woman suffering from heartburn and acid regurgitation was referred to our department for endoscopic examination. Endoscopy demonstrated more than one hundred tiny, rounded, elevated, white-yellowish lesions distributed throughout the entire esophagus (Fig. 1), which were identified during the histopathological examination as sebaceous glands (Fig. 2).

DISCUSSION

As of now, the histogenesis of ectopic sebaceous glands in the esophagus is unknown; whilst it could be a congenital abnormality, a majority of authors defined it like an acquired metaplastic process.

From a practical standpoint, it is an incidental finding that on occasion, as in our case, has been associated with gastro- esophageal reflux disease, not requiring treatment or en- doscopic follow-up and, and needing a differential diag- nosis from Candida infection and glycogen acanthosis.

El asiento de glándulas sebáceas extracutáneas en tejidos que derivan del ectodermo, como labios, cavidad oral, paróti- das y genitales externos, es un hallazgo frecuente. Por el contrario, la presencia de dichas glándulas en el esófago, que es un órgano de origen endodérmico, resulta una rareza: hay menos de treinta casos publicados, en su mayoría, con afecta- ción parcheada del esófago.

CASO CLÍNICO

Mujer de 50 años con pirosis y regurgitaciones ácidas que fue remitida a nuestro servicio para la realización de una en- doscopia oral programada. En dicha exploración se encontraron más de 100 lesiones milimétricas, redondeadas, sobreele- vadas y blanco-amarillentas, distribuidas de forma difusa a lo largo de todo el esófago (Fig. 1) e identificadas histológica- mente como glándulas sebáceas (Fig. 2).

DISCUSIÓN

En la actualidad se desconoce la histogénesis de la ectopia de glándulas sebáceas esofágicas; si bien podría tratarse de una anomalía congénita, la mayoría de los autores la definen como un trastorno metaplásico adquirido.

Desde un punto de vista práctico, se trata de un hallazgo incidental que en ocasiones, como en nuestro caso, se ha rela- cionado con la enfermedad por reflujo gastroesofágico, no requiere tratamiento ni seguimiento endoscópico y plantea diagnóstico diferencial con la candidiasis y la acantosis glucogénica esofágica.
