Paso del Norte Red De Promotora Network welcomes abstracts that highlight extraordinary jobs that Community Health Workers perform in their communities. Abstracts will be considered that show Community Health Workers (CHW) in
innovative and effective roles in the community they serve. The committee will review and choose abstracts for the conference.
Abstracts should be brief, emphasizing clearly their relevance to CHW practice and how the information presented can be applied in a universal setting.
Submission requirements: Limit abstract to 200 words
• One page
• Typed and double spaced
• Complete attached Presenter Contact Form Abstracts should include the following:
• Need for the program
• Goals and objectives of the program • Number of years program in operation
• Community Health Worker’s roles in the program
o How did the CHW’s roles in service delivery improve outcome care and education in the program? o What external and internal support mechanisms were available for the CHW in providing services
for the program?
o What innovative documentation/evaluation strategies were developed to capture the CHW’s impact on individuals, communities and systems?
o What role did CHW’s play in politics, policy and advocacy efforts related to the establishment of this program?

Paso del Norte Red De Promotora Network da la bienvenida a resúmenes que destacan los trabajos
extraordinarios que los trabajadores de salud de la Comunidad realizan en sus comunidades. Se tendrán en cuenta los resúmenes que muestran a los trabajadores de salud comunitarios (CHW) con funciones innovadoras
y efectivas en la comunidad que sirven. El comité revisará y elegirá los resúmenes para la conferencia. Los resúmenes deben ser breves, destacando claramente la relevancia de la práctica del CHW y cómo la
información que se presenta puede ser aplicada en un entorno universal.
Requisitos de presentación:

Community Health Worker Extraordinary Service Award
PurposeThe purpose of the Community Health Worker Extraordinary Service Award is to recognize and acknowledge outstanding service, dedication and excellence within the profession. All community health workers are eligible for nomination.
Nomination Process and Requirements
The nominee must be a community health worker
Must have a minimum of one year experience as a Community Health Worker Submit the nomination form
o Include the full name of the nominee on the nomination form. The description of the nominee’s contributions or achievements must be filled out, as well as how the nominee has influenced and contributed to their specific community service area(s).
One single spaced, typed page. Margins should be set at 1 inch, and font should be 12 point and include: o Explanation of work/job functions and duties
o Length of service as a community health worker o Description of the population the nominee serves
o Description of community involvement within the Community Health Worker role o Identify cultural sensitivity, diversity, and inclusion efforts

Selection of Award Recipients
The award winner will be selected by the award committee. The winner will be announced on October 13, 2014, during the Eleventh Annual Paso Del Norte Red De Promotora Network Conference at Double Tree Hotel in El Paso Texas. The award winner will receive a commemorative plaque, and his/her conference registration fee will be waived.
