Justificación
El linfoma difuso de células B grandes (LDCBG) es un cáncer agresivo con una mediana de supervivencia de <6 meses sin tratamiento. El cáncer es el crecimiento incontrolado de células humanas. El crecimiento de las células humanas normales se controla de múltiples formas. En las células cancerosas, una o más de estas formas de controlar el crecimiento no están funcionando correctamente o se han interferido para provocar un crecimiento incontrolable. Una forma específica en que las células cancerosas continúan creciendo es deshaciéndose de ciertas proteínas llamadas “proteínas supresoras de tumores” que normalmente causarían la muerte de las células cancerosas. El medicamento del estudio, selinexor, actúa atrapando las “proteínas supresoras de tumores” dentro del núcleo celular y, por lo tanto, hace que las células cancerosas mueran o dejen de crecer. probando actualmente en otros ensayos clínicos en pacientes con cánceres avanzados. Aproximadamente el 50-60% de los pacientes con LDCBG logran y mantienen una remisión completa después de la terapia de primera línea; 30-40% de recaída y 10% tiene enfermedad refractaria. El tratamiento estándar para el LDCBG recidivante refractario incluye quimioterapia de dosis alta seguida de un autotrasplante de células madre. Sin embargo, no todos los pacientes son aptos o elegibles para esta opción terapéutica. Por lo tanto, el propósito de este estudio es comparar los efectos buenos y malos del uso de selinexor junto con rituximab-gemcitabina-dexametasona-platino con el uso del tratamiento estándar de rituximab-gemcitabina-dexametasona-platino solo. La adición de selinexor al tratamiento habitual puede ayudar a tratar el cáncer, pero también podría causar efectos secundarios. Este estudio permitirá a los investigadores saber si este tratamiento diferente es mejor, igual o peor que el tratamiento estándar.

Criterios de Inclusión
Los criterios de inclusión son similares en las partes en fase II y III del estudio excepto cuando se indique de otro modo. I 1 Pacientes de edad > o = 18 años I 2 Con confirmación mediante estudio de anatomía patológica de LDCBG de novo o LDCBG por transformación a partir de un linfoma indolente diagnosticado previamente (p. ej., linfoma folicular. Los pacientes con linfoma de alto grado con reordenamiento de MYC, BCL2 o BCL6 son aptos (solo Fase 2). (se debe aportar documentación). I 3 Haber recibido al menos 1 pero no más de 2 líneas previas de tratamiento sistémico para el LDCBG (se debe aportar documentación). • La quimioinmunoterapia de rescate seguida de trasplante de células madre se considerará como 1 línea de tratamiento. • No se contará el tratamiento de mantenimiento como una línea distinta de tratamiento sistémico. • No se contará la radioterapia con intención curativa para LDCBG localizado como 1 línea de tratamiento. I 4 Enfermedad medible TEP positiva según los criterios revisados para la evaluación de la respuesta del linfoma con al menos 1 ganglio con diámetro máximo (DM) >1,5 cm o 1 lesión extraganglionar con DM >1 cm según la clasificación de Lugano de 2014) (se debe aportar documentación). I 5 No apto para recibir un TCMH ni tratamiento con CAR-T, según criterios clínicos objetivos determinados por el médico responsable del tratamiento. Los pacientes que no pueden recibir un TCMH por enfermedad activa pueden participar en el estudio (un máximo de un 10 % de pacientes inscritos en cada fase). La documentación sobre la ausencia de intención de administrar para proceder a recibir el TCMH o CAR-T la debe proporcionar el médico responsable del tratamiento. I 6 Función adecuada de la médula ósea en el cribado, definida como (se debe aportar documentación): • RAN > o = 1 x 10 9 /l • Recuento de plaquetas > o =100 x 109/l (sin transfusión de plaquetas < 14 días antes de D1C1) • Hemoglobina > o = 8.5 g/dl (sin transfusión de glóbulos rojos < 14 días antes de D1C1) I 7 Linfocitos circulantes < o = 50 x 10 9 /l. I 8 Función hepática y renal adecuada, definida como (se debe aportar documentación): • Aspartato transaminasa (AST) o alanina transaminasa (ALT) < o = 2.5 x límite superior de la normalidad (LSN), o < o = 5 x LSN en casos con compromiso conocido del hígado por el linfoma • Bilirrubina total en suero < o =2 x LSN, o < o = 5 LSN si se debe al síndrome de Gilbert o en casos con compromiso conocido del hígado por el linfoma • Aclaramiento de creatinina calculado (ACr) > o = 30 ml/min envasado en la fórmula Cockcroft-Gault . I 9 Estado funcional del Grupo Oncológico Cooperativo de la Costa Este (Eastern Cooperative Oncology Group, ECOG) < o = 2. I 10 Esperanza de vida estimada >3 meses en la selección. I 11 Los pacientes con LDCBG primario resistente, definido por la ausencia de respuesta o la aparición de una recidiva en los 6 meses posteriores al final del tratamiento de primera línea, podrán participar en el estudio (un máximo de un 20 % de pacientes inscritos en cada fase). I 12 Comprometerse a utilizar métodos anticonceptivos eficaces durante el estudio y durante 14 meses (pacientes de sexo femenino) y 11 meses (pacientes de sexo masculino) después de la última dosis del tratamiento del estudio. • Las pacientes con capacidad de concebir deben tener un resultado negativo en una prueba de embarazo en suero en la selección y comprometerse a utilizar métodos anticonceptivos muy eficaces durante todo el estudio y durante 14 meses después de la última dosis del tratamiento del estudio (excepto las pacientes sin capacidad de concebir: edad >50 años y amenorrea natural durante un período >1 año o con salpingoovariectomía bilateral o histerectomía previas). • Los pacientes varones sexualmente activos deben utilizar métodos anticonceptivos muy eficaces durante todo el estudio y durante 11 meses después de la última dosis del tratamiento del estudio. Los pacientes deben comprometerse a no donar semen durante el período de tratamiento del estudio ni durante los 11 meses posteriores a la última dosis del tratamiento del estudio.

E 4 Afectación conocida y activa del sistema nervioso central o meníngea por el LDCBG en el momento de la selección. E 13 Incapacidad para tragar comprimidos, síndrome de malabsorción o cualquier otra enfermedad o disfunción gastrointestinal (GI) que pudiera interferir en la absorción del tratamiento del estudio. E 14 Mujeres embarazadas en período de lactancia. E 15 Falta de capacidad o de disposición para firmar un formulario de consentimiento informado (FCI). E 16 Pacientes que, en opinión del investigador, estén significativamente por debajo de su peso corporal ideal.

Scientific Title
A Phase 2/3, Multicenter Randomized Study of Rituximab-Gemcitabine-Dexamethasone-Platinum (R-GDP) With or Without Selinexor in Patients With Relapsed/Refractory Diffuse Large B-Cell Lymphoma (RR DLBCL)
Rationale
The Diffuse Large B-Cell Lymphoma (DLBCL) is an aggressive cancer with a median survival of <6 months without treatment. Cancer is the uncontrolled growth of human cells. The growth of normal human cells is controlled in multiple ways. In cancer cells, one or more of these ways to control growth is not working properly or have been interfered with leading to uncontrollable growth. One specific way cancer cells continue to grow is by getting rid of certain proteins called “tumor suppressor proteins” that would normally cause cancer cells to die. The study drug, selinexor, works by trapping “tumor suppressing proteins” within the cell nucleus and thus causes the cancer cells to die or stop growing. Selinexor has previously been tested in humans to define a proper dose to be given. Selinexor is currently being tested in other clinical trials in patients with advanced cancers. Approximately 50-60% of patients with DLBCL achieve and maintain complete remission after first line therapy; 30-40% relapse and 10% have refractory

disease. Standard treatment for refractory relapsed DLBCL includes high-dose chemotherapy followed by autologous stem cell transplant. However, not all patients are fit or eligible for this therapeutic option. Hence, the purpose of this study is to compare any good and bad effects of using selinexor along with Platinum to using the standard of care treatment of Rituximab-Gemcitabine-Dexamethasone-Platinum alone. The addition of selinexor to the usual treatment may help treat your cancer, but it could also cause side effects. This study will allow the researchers to know whether this different treatment is better, the same, or worse than the standard of care treatment.
Main Objective
Phase 2: To compare efficacy (ORR - overall response rate) in patients with Relapsed/Refractory Diffuse Large B-Cell Lymphoma (RR DLBCL) treated with Rituximab-Gemcitabine-Dexamethasone-Platinum plus either selinexor 40 mg or selinexor 60 mg to R-GDP alone based on the Lugano 2014 Criteria.
Phase 3: To compare Progression-free survival (PFS) in patients with RR DLBCL treated with SR-GDP (selinexor + rituximab, gemcitabine, dexamethasone, platinum) followed by selinexor to PR-GDP (placebo plus rituximab, gemcitabine, dexamethasone, platinum) followed by placebo based on the Lugano 2014 Criteria.

Inclusion criteria
Inclusion criteria are similar in the Phase 2 and Phase 3 portions of the study except where noted. I1 Patients > or = 18 years of age. I2 Have pathologically confirmed de novo DLBCL or DLBCL transformed from previously diagnosed indolent lymphoma (eg, follicular lymphoma). Patients with high grade lymphoma with myc, bcl2 and/or bcl6 rearrangements are eligible (for Phase 2 only). (Documentation to be provided). I3 Have received at least 1 but no more than 2 prior lines of systemic theraphy for the treatment of DLBCL (Documentation to be provided). • Salvage chemoimmunotherapy followed by stem cell transplantation will be considered as 1 line of therapy; • Maintenance therapy will not be counted as a separate line of systemic therapy; • Radiation with curative intent for localized DLBCL will not be counted as 1 line of therapy. I4 PET positive measurable disease per the revised criteria for response assessment of lymphoma (with at least 1 node having longest diameter (LDi) >1.5 centimeter (cm) or 1 extranodal lesion with LDi >1 cm (per the Lugano Classification 2014) (Documentation to be provided). I5 Not intended for HSCT or CAR-T cell therapy based on objective clinical criteria determined by the treating physician. Patients who cannot receive HSCT due to active disease are allowed on study (up to 10% of patients enrolled in each Phase). Documentation on lack of intention to proceed to receive HSCT or CAR-T therapy must be provided by the treating physician. I6 Adequate bone marrow function at screening, defined as (Documentation to be provided): • ANC > o= 1 x 109/L • Platelet count > or = 100 x 109/L (without platelet transfusion <14 days prior to C1D1) • Hemoglobin > or = 8.5g/dL (without red blood cell transfusion <14 days prior to C1D1) I 7 Circulating lymphocytes < or = 50 x 109/L. I 8 Adequate liver and kidney function, defined as (Documentation to be provided): • Aspartate transaminase (AST) or alanine transaminase (ALT) < or = 2.5 x upper limit of normal (ULN), or < or = 5 x ULN in cases with known lymphoma involvement in the liver; • Serum total bilirubin < or =2 x ULN, or < or = 5 ULN if due to Gilbert syndrome or in cases with known lymphoma involvement in the liver; • Calculated creatinine clearance (CrCl) > or = 30 mL/min based on Cockcroft-Gault formula. I9 Eastern Cooperative Oncology Group (ECOG) performance status of <=2. I10 An estimated life expectancy of >3 months at Screening. I11 Patients with primary refractory DLBCL defined as no response or relapse within 6 months after ending first-line treatment will be allowed on study (up to 20% of enrolled patients in each phase). I12 Agree to effective contraception during the duration of the study with contraception use for 14 months for female patients and 11 months for male patients after the last dose of study treatment. • Female patients of childbearing potential must have a negative serum pregnancy test at Screening and agree to use highly effective methods of contraception throughout the study and for 14 months following the last dose of study treatment (except patients with non-childbearing potential: Age >50 years and naturally amenorrhoeic for >1 year, or previous bilateral salpingo-oophorectomy, or hysterectomy) • Male patients who are sexually active must use highly effective methods of contraception throughout the study and for 11 months following the last dose of study treatment. Male patients must agree not to donate sperm during the study treatment period and for 11 months following the last dose of study treatment.
Exclusion criteria
Exclusion criteria are similar in the Phase 2 and Phase 3 portions of the study except where noted. E1 DLBCL with mucosa-associated lymphoid tissue (MALT) lymphoma, composite lymphoma (Hodgkin’s lymphoma + non-Hodgkin’s lymphoma [NHL]), DLBCL transformed from diseases other than indolent NHL; or primary mediastinal (thymic) large B-cell lymphoma (PMBL); T-cell rich large B-cell lymphoma. E2 Previous treatment with selinexor or other XPO1 inhibitors. E3 Contraindication to any drug contained in the combination therapy regimen (SR-GDP). E4 Known active central nervous system or meningeal involvement by DLBCL at time of Screening. E5 Use of any standard or experimental anti-DLBCL therapy (including nonpalliative radiation, chemotherapy, immunotherapy, radio-immunotherapy, or any other anticancer therapy) <21 days prior to Cycle 1 Day 1 (Prednisone <30mg or equivalent are permitted; palliative radiation is permitted only if on non-target lesions). E6 Any AE, by Cycle 1 Day 1, which has not recovered to Grade <=1 (Common Terminology Criteria for Adverse Events [CTCAE], v. 5.0), or returned to baseline, related to the previous DLBCL therapy, except alopecia. versus-host disease after allogeneic (or cannot discontinue GVHD treatment or prophylaxis) or CAR-T cell infusion <90 days prior to Cycle 1. E9 Neuropathy Grade > or = 2 (CTCAE, v. 5.0). E10 Any life-threatening illness, medical condition, or organ system dysfunction which, in the Investigator’s opinion, could compromise the patient’s safety, or being compliant with the study procedures. E11 Uncontrolled (i.e., clinically unstable) infection requiring parenteral antibiotics, antivirals, or antifungals within 7 days prior to first dose of study treatment; however, prophylactic use of these agents is acceptable (including parenteral). E12 Patients with active hepatitis B virus (HBV), hepatitis C virus (HCV) or human immunodeficiency virus (HIV) infections. Patients with active HBV are allowed if antiviral therapy for hepatitis B has been given for >8 weeks and viral load is <100 IU/mL prior to first dose of study treatment. Patients with known history of HCV or found to be HCV antibody positive on screening, are allowed if there is documentation of negative viral load per institutional standard. Patients with HIV who have CD4+ T-cell counts > or = 350 cells/microL, negative viral load per institutional standard, and no history of AIDS-defining opportunistic infections in the last year are allowed. E13 Inability to swallow tablets, malabsorption syndrome, or any other gastrointestinal (GI) disease or dysfunction that could interfere with absorption of study treatment. E14 Breastfeeding women. E15 Inability or unwillingness to sign an informed consent form (ICF) E16 In the opinion of the Investigator, patients who are significantly below their ideal body weight.
