• the name of the supply, together with a unique identifier; • the type of source. There are three categories of sources: (1)

surf ace waters (ri ver s, streams, lak es, reser vo irs); (2) g round waters (spr ing s, we ll s, boreho les) th at are not inf lu ence d b y surf ace waters; and (3) mixed so urces w hic h are g round waters that are inf lu enced b y surf ace wa ter. For g round wate r the g eolog ic al strat a shou ld be re corde d) ;

• the geographical location using a grid reference – this means the 12 f ig ure ordnan ce sur ve y g rid ref ere nce (East ing and North ing ) of the loca tion of the source, a s clos e as po ssi bl e to its k no wn locat io n (a s the prec ise locat io n of the source may not be ob vi ous);

• an estimate of the number of people;

• an estimate of the average daily volume of water supplied in cub ic metres (us ing 0.2m3/da y per pers o n, whi ch i s 200 l itres per perso n per da y);

• the type of premises supplied (for example private domestic d we l li ng s, hotels, b e d and bre ak f ast establ ishme nts, schoo ls,

• detail of any treatment process, together with its location

(exampl es of treatment proces ses inc lud e col le ctio n chamb ers, ble nd ing , coag ulat io n and f loccu lat io n, cl arif icati on,

settlement, se dime nt ation, a erati on an d o xid atio n, f iltratio n, ion e xcha ng e, membranes, d isi nf ection); and

• the name of the HPU in whose area the supply is located (the address, te le pho ne/fax/ e-mai l, and c onta ct person) .

2.3.6.2 T he loca l aut horit y must revie w and upd ate that in it ia l record at lea st once a year an d shou ld d o so as s o on as pract ica l af ter an y sig nif ica nt chang e i n circumstan ces (f or examp le i nsta ll ati on of a ne w treatment pr oce ss). For supp l ies to sing l e d wel l ing s and supp l ies of 10m3/da y or less that ar e not part of a commercial or pub li c acti vit y, l oca l author itie s must upd ate the record s whe n the y are a ware of any ch ang es (suc h as a ne w r isk assessme nt, or result of monitor in g or other inf ormation about the s upp l y). T his inf ormat ion c an be g athered dur ing a g eneral vis it to the supp l ies (not re late d to a water sup pl y) o r as a desk top e xer cise throug h q uestion na ir es, etc. T his record must be k ept f or at least 30 years.

• a plan and description of the supply – this schematic plan f orms part of the risk assessment a nd sh oul d sho w/des crib e the source, the tre atment and d istri buti o n net work (inc lu di ng ins pect ion ch ambers or storag e tank s) ;

• the monitoring programme for the supply – (number of samples pla nne d f or the year, number of samples t ak en during the ye ar and whether s upp l y clos ed/or was no lon g er in operat ion or ne w su pp l y so urced duri ng the ye ar);

• the risk assessment (the result of the risk assessment); • the date, results and location of any sampling and analysis

relat ing to that supp l y, an d the reas on f or tak ing the sample (routin e prog rammed compl ian ce samp le (dist ing uis hi ng wheth er the samp les is an a ud it or check monitor ing sample) ,

• the results of any investigation (of a failure to meet a standard or ind ic ator paramet er va lu e) und ertak en in acc ordan ce with the Reg ul ati ons; whe ther the supp l y h as been d etermin ed as a potent ia l risk to human hea lth or n ot, and whether th ere is an inf ormal or f ormal ac tion p la n. T his cou ld cover impro vements to the source ( lik e f enci ng , di vers ion d itc hes), impro vement work s to col lect ion c hambers, storag e ta nk s or inspect io n chambers, the d istr i butio n net work (such as pip e work or va l ues), and/ or an y t reatment proces ses (either o n the sup p l y or at ind i vidu al prop erties) ;

• any authorisation (granted by the local authority for that supp l y) – the p aram eter, maximum conc entratio n or va lue author ised, the rem e dia l act ion and d urat ion;

• any notices served under section 80 of the 1991 Act (or reg ulati on 18) – brie f details of the actio n req uired b y th e notic e and wheth er the not ice b een comp il ed with b y th e date spec if ied i n the not ic e;

• any action agreed to be taken by any person under the

Reg ul atio ns ( wh ethe r action ag ree d inf ormall y or b y means of an auth oris atio n or a notice) ;

• any request for the local authority to carry out sampling and ana l ys is, und ertak e a risk assessment or g ive a d vice (reas on f or req uest – such a s compl ai nt, prospec ti ve ho use p urcha se, compl ai nt f rom a tenant ); and

• a summary of any advice given in relation to the supply (and who the ad vice was g iven to) .

2.3.6.4 T he loca l aut horit y must retain the inf ormation in the rec ord relat ing to the risk assessments a nd sam pl ing and a na l ys is f or at lea st 30 years and it must retain the oth e r inf ormation f or at least f ive ye ars.
