providers you can use to get services in the Long Term Care program.
These providers have agreed to provide long term care services for you. Services will be approved
by your Case Manager. You may choose any provider from this list. You may change providers at
any time. If you would like to change providers, call our Member Services Department at
1-877-211-1999.
Some network providers may have been added or removed from our network after this directory
was printed. All providers may not be taking new members. To get the most up-to-date list of
