Staying in a host family is part of the intercultural experience and facilitates the pupil's integration into the foreign country and culture. It is therefore important that the pupil is not treated as a guest or lodger, but is integrated into family life to the greatest extent possible. The trainee is a young person who may have little or no previous experience of the culture and attitudes of your country.
During his/her stay with your family, the pupil is under your authority, but you are also part of a team. The host school has appointed a mentor for the pupil, who you can turn to for information, advice or simply to discuss any aspect of the pupil's stay in your home. I agree that the sending school will pass on the details about my child, as stated in the pupil registration form, to the receiving school, and that the receiving school will pass on the relevant details to the family where my child will be hosted.
I understand that basic information about my child will also be communicated to the relevant National Agencies responsible for Erasmus+. I do not consent to such use of my child's photos, films and video footage. I understand that this part of the scholarship will be transferred to me (to be transferred to my child) or directly to my child by the sending school.
I understand that the data in this form may also be communicated to the sending and receiving National Agencies responsible for the Erasmus+ programme.

I agree that the envelope with this form can be given to a doctor treating my child abroad where it is medically necessary. If necessary, I agree to disclose all relevant information regarding my child's health to the receiving school and host family.

Are there any other aspects to consider when matching your family with a pupil. Give information about the personality, recreational activities practiced and any other interests of the family members. Describe your home and give more details about how you intend to accommodate the pupil in your home (his/her personal space/room).
Please describe your reasons for volunteering as a host family, what you expect to gain from participating in this program, and how you plan to support the student during his/her stay. I/We, the undersigned, certify that all information provided in this form is complete and correct. I/We are not aware of any obstacles that would prevent us from successfully hosting a student.
I/we, the undersigned, authorize the receiving school to communicate the personal data indicated in this form to the sending school, the student and his/her family for purposes of planned long-term study mobility. I/We understand that the information contained in this form may also be communicated to the sending and receiving National Agencies responsible for the Erasmus+ programme. I/We understand that the decision to host a student must be made on the basis of curiosity and open-mindedness, therefore no payment of host families is provided.
I/we, the undersigned, are aware that the support we provide to the learner will be crucial to the success of his/her mobility experience. I/We understand that before we are accepted to host a student, (i) a school representative will come to us for a visit, (ii) we will need to provide a final criminal record check for any adult living in the household and (iii) we will need to sign the Host Family Card.

This part can also be accompanied by a work portfolio (written compositions, tests, artistic work, etc.) realized by the student during the stay. What the student achieved in the following areas compared to the goals set out in the learning agreement. This evaluation must be written by the mentor/contact teacher at the receiving school and provide an overall assessment of the student's achievements.
It should also mention the added value of the stay in the receiving school and host family abroad for the pupil (i.e. intercultural skills, personal development, interpersonal skills, etc.).
