La presencia y grado de déficits cognitivos a largo plazo depende de muchos factores. Por ejemplo, el funcionamiento premórbido, la edad del paciente, aspectos demográficos, condiciones comórbidas, la localización y el volumen de la lesión, el desarrollo de epilepsia, entre otros (Sachdevet al. 2006; Jokinenet al. 2005; Steinlin, Roellin y Schroth, 2004). Además, también influye la causa subyacente de la EVC, puesto que puede haber riesgo de futuros eventos y en potencia todavía afecta la cognición. Por otra parte, se considera que los déficit cognitivos por lo general forman una curva en forma de U en relación a la edad en que ocurre la EVC, pues los déficit más persistentes y severos ocurren en niños pequeños y en adultos mayores (Duvalet al. 2008; Ballardet al., 2002).
En niños que han sobrevivido a un evento vascular cerebral isquémico, se ha observado que los déficits neuropsicológicos que más persisten son las dificultades de atención y concentración, así como la velocidad de procesamiento (Steinlinet al., 2004). Además, un estudio encontró que cerca de la mitad de niños que sufrieron un evento vascular cerebral hemorrágico presentan déficits cognitivos, pero también baja autoestima, dificultades emocionales y conductuales (Blomet al., 2003). También se ha sugerido que después de un evento vascular cerebral, los infantes tienen una alta probabilidad de presentar dificultades académicas y de necesitar servicios de educación especial (Steinlinet al., 2004). Los efectos de un evento de este tipo en el funcionamiento neuropsicológico suelen ser más extensos que los déficit asociados con lesiones específicas. Además, esto es en particular cierto en niños que han presentado un evento vascular cerebral, porque ocurren mientras su cerebro se desarrolla y la plasticidad permite la reorganización cerebral. Por esta razón, se debe considerar que mientras en los adultos los déficit cognitivos son facilidad evidentes, en los menores éstos pueden pasar desapercibidos porque es posible que no haya una pérdida funcional inmediata, sino podrían tener fallos en el desarrollo de habilidades mientras crecen (Smithet al., 2010). La reintegración al hogar, la escuela o el trabajo podría convertirse en un verdadero reto para los pacientes que han sufrido cualquier tipo de EVC, ya sea déficit sensorial, motor, cognitivo o emocional, ya que éstos pueden limitar en forma severa sus habilidades y dificultarles la capacidad para trabajar, conducir un auto, cuidar de sus propios hijos o incluso, de vivir de manera independiente (Melkas, Jokinen, Hietanen y Erkinjuntti, 2014).
Por si fuera poco, se incrementan demandas sanitarias puesto que deben asistir con regularidad al médico o a la rehabilitación, con los gastos económicos que esto conlleva. La carga y el agobio de esta situación recae en la familia y a veces sólo en pocos miembros de ésta, por lo que pueden padecer estrés y presentar problemas emocionales tales como depresión o ansiedad (Forster, 2005). Por tanto, también se recomiendan intervenciones y asesorías centradas en la familia.
Una identificación temprana de las alteraciones cognitivas causadas por la EVC es una oportunidad para llevar a cabo intervenciones como el control de los factores de riesgo vasculares, con el fin de minimizar, detener e incluso revertir en algunos casos el deterioro cognitivo. Por lo tanto el objetivo no sólo es el diagnóstico oportuno de los diferentes síndromes o deterioro causado por evento vascular, sino prevenirlos. No obstante, es importante que de una manera multidisciplinaria se busque alcanzar el mayor grado de funcionalidad del paciente.
