10. DISCUSIÓN

10.9 Limitaciones

Esta es una investigación de tipo caso único con replicación entre sujeto lo cual limita la generalización de los resultados. Otro factor importante en la limitación de la generalización de los resultados es la proporción alta de abandono. También hay que considerar que las escalas eran del tipo auto-reporte, y en los registros diarios, basados en auto-observación registrando lo observado, se observaron varias dificultades. Por ejemplo, por motivos metodológicos, se tuvo que decidir sobre las conductas a seguir en los registros de manera apresurada. No se tuvo control sobre si los registros se llenaron de manera diaria o con menor frecuencia. También se observó que dos de los participantes empezaron a dejar vacíos en sus registros cuando consideraron que “no era problemático”

el impulso o la emoción. Fue una lástima que no se logró tener registros sobre la práctica diaria de las habilidades, pero como mencionan Neaciu et al. (2010), hay varias debilidades en el uso del registro diario de habilidades. Entre ellas mencionan que los registros diarios son herramientas clínicas – no de evaluación, así que la falta de registro puede significar que practicaron la habilidad sin registrarlo; que los participantes en entrenamiento de habilidades entienden que el uso de habilidades es deseable y es posible entonces que se registra sobre estimaciones de uso; y que es difícil separar si un aumento de uso reportado

es debido a un incremento real o un incremento en el conocimiento del “idioma DBT”.

Debido a que los resultados de esta investigación mostraron poco incremento en el uso de habilidades DBT medido por la escala DBT-WCCL, en combinación con lo propuesto de la importancia en el incremento del uso de habilidades, sería importante en investigaciones futuras comparar registros de uso con la escala DBT-WCCL.

Otro problema es que las escalas usadas para evaluar el cambio no estuvieron estandarizadas en población normal. Por ejemplo la BSL-23 tiene correlación con el diagnostico BPD hecha con entrevistas semi-estructuradas (Bohus et al, 2007) pero no se ha usado la escala en población sin diagnóstico, ni en otros diagnósticos. En este sentido se puede decir que hubo cambios significativos pero no se puede decir si fueron clínicamente

relevantes, es decir, si entraron en remisión del diagnóstico al entrar en el rango de una población normal. Esto aplica también a la escala DERS donde no se sabe si los

participantes mostraron una desregulación emocional afuera de “lo normal” y que el

cambio tenía relevancia clínica. En la escala FFMQ, en la validación al idioma español, Cebolla et al. (2012) usaron un tamaño de población clínica igual a la población no clínica pero en su artículo no presentaron los resultados de cada grupo ni tampoco calcularon la confiabilidad test-retest. Debido a esta falta hay que analizar los resultados de esta escala de manera cuidadosa. En la escala DBT-WCCL se considera que este problema es de menor importancia dado que el objetivo de la intervención era incrementar el uso de habilidades DBT y disminuir el uso de habilidades disfuncionales. En este sentido cualquier cambio significativo es relevante. Mirando la sub-escala de uso de habilidades disfuncionales hay que considerar la falta del cálculo de confiabilidad test-retest. Dadas estas limitaciones se recomienda para investigaciones futuras trabajar las escalas encontrando dónde se encuentran los puntos críticos entre varias poblaciones, por ejemplo la normal y la población clínica, al igual que la confiabilidad del instrumento en mediciones repetitivas (test-retest).

10.10 Conclusiones

La intervención de entrenamiento de habilidades como fue planteada en esta investigación parece tener problemas de deserción altos. Hay que considerar para qué tipo de problemática esta intervención se podría considerar y para qué tipo de persona no es probable que sea una intervención recomendable. En esta investigación, basada en una población participante de método de conveniencia y con un N pequeño, se pudo observar que los participantes que continuaron en la intervención tenían un funcionamiento más elevado (según datos descriptivos). Es posible que con un menor nivel de problemática fuera más probable que una intervención estructurada, dirigida al cambio y de corta duración, sea más aguantable y más útil para los participantes, mientras que para individuos con mayor nivel de problemática y afectación de funcionamiento este tipo de intervención no fuera recomendable. Como está planteada, la DBT estándar generalmente hace contratos para intervención con un año de duración, conteniendo las cuatro modalidades (por ejemplo Neacsiu et al., 2010). A diferencia de esto, esta investigación consistió en un entrenamiento de habilidades durante 14 sesiones.

Aun así, los resultados muestran que la intervención tenía efecto comparando mediciones pre y post, y pre y seguimiento 2, y tenía efecto sobre conductas problemáticas en los participantes de serie 1, uniéndose a otros investigaciones mostrando efecto de un entrenamiento corto de habilidades DBT (Soler et al., 2009; Soler et al., 2012). Se hipotetiza, que el orden del entrenamiento de las habilidades influye sobre el efecto que tiene el entrenamiento completo, igual al efecto que tiene el entrenamiento de mindfulness. También se observa mayor nivel de deserción del serie 2 que serie 1. Una posibilidad es que el orden del entrenamiento podría relacionarse con la deserción.

Otra conclusión, dando soporte a investigaciones de tipo caso único en el futuro, es que se muestra que el entrenamiento tenía efecto variado en todos los cuatro participantes. En este sentido es importante en el trabajo clínico ajustar la intervención de manera idiográfico en vez de continuar con intervenciones de protocolo.

En el área investigativo sobre el efecto de orden y el efecto diferenciado entre varios componentes de una intervención todavía se encuentra en el inicio de las investigaciones y es necesario seguir con estos tipos de estudios en el futuro.
